• Notary: No requirement in statute that signature of principal upon POA be acknowledged by a notary; however, it is highly recommended that the signature of principal be notarized. The standard MedLawPlus.comŽ POA form does have a signature block for the notary.
  
• Witnesses: There is no requirement of witnesses to the signature of principal.
