Question: Hello, I have a couple questions. First, I had breast augmentation/lift on 03/31/06. I have very bad rippling and can feel the implant the whole way around the left breast which causes me pain. I went back to the original plastic surgeon which tells me everything will get better, my 2nd opion said the implant should have been placed under the muscle (I am 5'6" 105# have thin skin, right now I have silicone, textured, overs) to try to hide some of the rippling. 3rd ps says it wouldn't have mattered. Any suggestions? I am confused. Also I have a revision how long should I wait?

Response: First, I am a little confused because only women who have had implants before can generally obtain silicone implants. Did you have implants prior to the first surgery that you specify in your question? That not withstanding, I tend to agree with your plastic surgeon and the third consult. The outer part of your breast is not covered with muscle, even when the implant is placed under the muscle (think of a body builder and how the large chest muscle curves, just below the male nipple and away from the underarm area). This is the muscle that is used to cover the top portion of the implant. If you have rippling there, it may have been better to place the implant under the muscle but, if your muscle is thick or you work out a lot, submuscular placement can be very problematic. If your rippling is on the front of your implant (rather than the side), your muscle is thin and you are not an exercise fanatic, I would suggest that you have the implants changed to under the muscle in a few months or, better yet, if you can, wait for cohesive gel implants to become available which have a much lower incidence of rippling (there are a few centers placing these now under study protocols and you may be a candidate - ask your plastic surgeon about this). Much of the early rippling may improve with time but you are 5 months out and most of the swelling should be gone by now. Again, a lot is dependent upon whether or not your first surgery was the one in March.
