Question: OK, so I have been told by several plastic surgeons that my Malar bags can NOT be removed with a lower Blepharoplasty. Then will a mid face lift work? One Doctor said they can NEVER be removed as it's an Edema that comes & goes. He said he could remove the abundance of lose skin, which will leave a scar but it's not a 100% fix. What can I do. I'm a 51 year old female in great shape except for these awful bags and lose skin that is wrinkled up. These came about after several attempts on a rhinoplasty & revisions on my nose. Also have had a face lift several years ago and upper bleph. Botox doesn't help,it makes it worse. I am desperate,please help.

Response: Malar bags may be related to a lower eyelid problem, or may be an issue in and of themselves. When related to a lower eyelid problem, sometimes, lower eyelid surgery may be an option. If there is no lower lid issue, then a more direct approach to the bag is needed. Occasionally, resurfacing can help a bit. Also, depending upon your facelift and how it was performed, the bags may be a part of the aging process. In your particular situation, it is impossible to outline your options without an examination. You need to find a surgeon who understands your goals and your anatomy and is willing to help you. You also need to understand that you amy get a result that is something different from your goal. Finally, if the problem is related to water in the tissues (edema) surgery is not a good option.
